$500 Toys "R" Us Gift Card with Enrollment

New enrolling families:  Choose to enroll your family at Children's Discovery Academy by November 30th, 2012 and you'll receive a $500 Toys "R" Us gift card!

Must print this page and present it with enrollment paperwork before first day of attendance.  Cannot combine with any other offer or discount.  One gift card per family.  Gift card will be given after 30 days of attendance.  You may choose to have a $500 credit applied to your tuition at Children's Discovery Academy instead of receiving a gift card.

New enrolling family must write your name on their enrollment application for you to qualify for the referral gift.  One gift card will be given for each family you refer to Children's Discovery Academy.  Gift card will be given after the new enrolling family completes 30 days of attendance.  You may choose to have a $500 credit applied to your tuition at Children's Discovery Academy instead of receiving a gift card.
